Incident Investigation & Support
Root cause analysis, corrective actions, and documentation that holds up—built to prevent repeat incidents and streamline internal/legal reporting.
What we do
SSCS supports incident response by gathering facts quickly, documenting the timeline, conducting interviews, and identifying root and contributing causes. We focus on repeat-prevention: corrective actions that can be owned, tracked, verified, and closed.
Root cause analysis approach
We use practical root cause methods appropriate for the incident severity and data available (e.g., causal factor analysis, “5 Whys,” barrier analysis, contributing factor mapping). Deliverables are written in clear language that leadership can understand and act on.
Multilingual investigations
Interviews and investigation support can be conducted across multiple languages to reduce miscommunication and improve accuracy. Languages supported: English, Spanish, Russian, Mandarin Chinese, German, and Georgian.

Streamlined reporting and workflow options
Where needed, we can structure investigation outputs to align with modern EHS management workflows (e.g., Origami-style reporting, routing, and role-based visibility). This allows investigation results to communicate clearly with:
- Legal / Claims
- Compliance
- Operations leadership
- Safety/EHS teams
- HR (as applicable)
Deliverables
- Incident timeline and fact summary
- Root cause / contributing factor analysis
- Corrective action plan (owners, due dates, verification steps)
- Investigation report formatted for OSHA/insurance documentation needs
- Optional leadership brief (what happened, why, what changes now)
How it works
- Intake + immediate fact gathering
- Interviews + site review + evidence/document collection
- Findings + corrective actions + closeout support
